Our program offers a variety of individualized services and supports to ensure the health and safety of the individuals who live in their own homes within their community.

We offer up to 24-hour staff supported care and services. This may include rehabilitation, nursing, behavioral support, assistance with daily living skills, home care, personal care, and community inclusion. Community involvement and social interaction empowers individuals to live fulfilling independent lives.

This program is funded through the Department of Mental Health.
