Our program provides community inclusion for individuals with disabilities to fully participate in the community and enhance their independent living skills. Choices staff support the individual’s choices in learning or utilizing their social skills while participating in community-based activities. Planned activities may include shopping, socialization, volunteering, or daily life skills.Personal Assistance
Personal assistance may include bathing, toileting, transfer and ambulation, skin care, grooming, hygiene, use of adaptive equipment, household chores, or any person-centered plan designed to help individuals with their daily life skills.Community Networking is funded through the Department of Mental Health and/or local SB-40 Services.

Employment Services
Employment Services help secure competitive employment and on-the-job supports for individuals eligible for services. Services are available to individuals with disabilities to achieve their employment goals through career exploration, and extensive one-on-one evaluation to ensure successful outcomes for the individual.Tailored services include career counseling, training, job placement assistance, assistive technology, and many other services. Job retention, employee/employer relationships, and continuing supports are also focuses of the program.
Employment Services are funded by Vocational Rehabilitation and/or the Department of Mental Health.
